Patient Registration

After you schedule your first appointment, we will email you a link to our online forms.  These forms  gather contact and insurance information as well as information pertaining to your medical history.  Please complete the forms before you arrive for your first appointment. 

IMPORTANT: If you have had x-rays taken at another dental office within the past few years, please alert our office when you schedule your New Patient Appointment.  Our office staff will help you determine if you should have the x-rays forwarded to our office in advance of your appointment.  It is important that all recent x-rays are available for your New Patient Visit.   If additional x-rays are necessary, they can be taken at our facility.
